Chosen First Name Change for

LMSD's Unofficial Pupil Records

As stated in the LMSD policy and administrative regulations P259/R259, the District shall permit a student or parent/legal guardian to request a change of name so that the student may be registered in school under a name that corresponds with the student's identity without obtaining court order or without changing the student's official records. This request shall be made via this electronic form.

This form can only be used to change a student's first name.

This form can be submitted by the student requesting the name change or by the student's parent/legal guardian.

Efforts will be made to use the student's name whenever possible but the student's legal name will still be in the system and may be used by school employees and electronic programs in some circumstances. This form does not substitute for an official name change, which would require a court order. *
