To undertake course planning for Semester 2.
To provide information to staff in Colleges and Departments in order to prepare a plan for you for Semester 2 while we are under Covid-19 Alert Level 2.
To provide information to staff in your Hall of Residence if you choose for this information to be shared with them. If you do not wish for this information to shared, please select “prefer not to say” when asked about living in a hall. You information will only be provided to your hall and it will be used to support the hall to develop a plan that supports your health.
If you have requested contact by one of our student support teams, the information in this form will also be shared with them to assist them to understand your needs and to contact you.
To undertake high-level reporting within the University or to external organizations where the University agrees to provide the information. No information that could identify a person will be provided in this circumstance.
To identify your Semester 2 Course Coordinators and email addresses visit here. Search for your course and scroll down the course information page to find the name of your course coordinator. Click on their name to view their email address.
If the instructions refer you to a Head of Department or there is no Course Coordinator listed, tick the box to say that you cannot identify your Coordinator.
If you are a postgraduate research student you should put the name of your Postgraduate Coordinator - a list of these can be found here.
You do not need to provide detailed personal medical information when answering the following questions. Tell us about what you are able to do in Semester 2 relating to your study – e.g. 'I am happy to attend all my classes, I can attend small lectures, I’m not able to be in a customer facing role for my work integrated learning, I need to remain at home at all times because of my health, I’m comfortable with attending a tutorial with physical distancing but I am worried about attending my large RECN300 lecture'.
