SOC Care Review Referral Form Care Review Team Referral & Scheduling ProcessAny member from the Child and Family Team can make a referral to Care Review, (the parent, the youth, Parent/Family Advocate, Department of Social Services, Department of Juvenile Justice, Community Support worker, Intensive In-home worker, Multi-Systemic Therapy worker/therapist, and group home workers), they are asked to complete and submit an electronic referral form before attending a Care Review meeting. Please limit to a few sentencesNote: Please submit the consumer’s most recent person centered plan, comprehensive clinical assessment, and psychological testing, if applicable via Provider Direct. Please bring five copies of each document to the scheduled care review. If a parent is making the care review referral, please make arrangements with the System of Care Coordinator to get copies of the clinical assessment and/or person centered plan.
While anyone can request a Care Review Team, it will be essential that such request is communicated with the team immediately. You understand that the information you have provided for the child/client is accurate and will be treated as confidential. You will also submit any additional required documents. Please remember that the legally responsible person and the youth needs to be present for the care review. Please be aware that if your team is 10 minutes late there is a possibility that the care review meeting will have to be cancelled and rescheduled. |
